We have two options for consents - outlined below:
1. Paper (Offline) Consent Forms
We encourage you to use our offline consent process to gather consents from residents or their guardians. Please distribute the form (attached at bottom of this page) to guardians or self-consenting Test Takers. If you have any staff under the age of 18, they will also need parent/guardian consent and should be included in this process. It is the responsibility of the organization to ensure all Test Takers participating in testing have a completed consent prior to testing and that consented Test Takers are uploaded to the Concentric Portal. When utilizing paper consents, Test Takers will need to be added to the website through a bulk upload process. Attached at the bottom of the page are also slides with screenshots on how to complete this process.
If using offline consents, please maintain these records per organizational policy. Concentric does not need a copy of paper consents.
Test Takers included in this upload will not have accounts on Concentric’s website and Test Champions will need to view and communicate their test results.
2. Digital Consent Forms
We also have the option for self-consenting Test Takers (residents or staff over 18) to complete a digital consent form. The digital consent form is available in English and Spanish.
A Test Champion will need to take two steps to collect consents for these Test Takers:
- Distribute this link: (https://testcenter.concentricbyginkgo.com/invite-by-code). This enables a Test Taker to create an account and provide consent.
- Provide an access code to the Test Taker to associate the individual with your organization. Need help setting up your access code? Read more here.
Test Takers completing the digital consent forms may create a personal account on our website and will have access to their individual test results. Test Champions will need to provide the Test Taker with the access code to create their account. When creating a personal account, the website will ask them to provide individual consent. Please note that Test Takers using this option will not be able to participate in testing until they have activated their portal account by accessing the email invite they will receive after providing consent.
How will I know if my Test Takers are consented?
When a Test Taker completes the online consent process, this automatically adds the Test Taker to your testing portal. Test Champions should ensure that a Test Taker has consent completed by checking that the "Profile Status" is "Complete" on the Test Taker screen under Account Information.
